1. Definitions

The following words and phrases shall have the meanings ascribed to them in this section, unless the context clearly indicates otherwise:

“Client” means the person who has contracted with ESP for the provision of services.

“ESP” means the entity providing the services.

“Service” means the provision of support and assistance to a Client with a disability.

2. Scope of Services

ESP will provide services to Clients who have a disability as defined by the Americans with Disabilities Act of 1990.

3. Eligibility

ESP services are available to any person who has a disability and requires assistance.

4. Application

To apply for services, a Client must complete and submit an application form provided by ESP.

5. Service Fees

Fees for services will be determined on a case-by-case basis, and will be based on the nature and extent of the services required.

6. Confidentiality

The privacy of all Clients will be protected at all times. Information about Clients will not be released to any third party without the express written consent of the Client, except as required by law.

7. Termination of Services

ESP reserves the right to terminate services at any time, for any reason. If services are terminated, the Client will be given written notice of termination and the reasons for the termination.

8. Changes to Terms and Conditions

ESP reserves the right to change these terms and conditions at any time. Changes will be effective immediately upon posting on the ESP website.
